Our health-related support focuses on practical assistance that helps people manage day-to-day health needs at home in line with an agreed care plan. For many people receiving home care, staying well is closely connected to routines such as taking medication correctly, eating and drinking enough, keeping comfortable, and noticing early signs that something may be changing. We provide support that fits within home care practice and is delivered by staff who are trained and supported to work safely.

Medication support is provided only where it has been assessed and agreed. Depending on the individual’s plan, this may involve prompting and supervision, support to help someone take their medication as prescribed, and medication administration where appropriate arrangements are in place. We treat medication safety as a core part of quality care: staff follow the agreed instructions, record support provided, and raise concerns promptly when something does not look right, such as missed doses, side effects, confusion, or changes in the person’s ability to manage medicines safely.

We also support people living with long-term conditions by helping them keep to daily routines that support stability and comfort. This might include encouraging hydration and nutrition, supporting rest routines, assisting with mobility as agreed, and paying attention to changes that may need follow-up. Where a person’s needs involve other professionals, we work alongside families, GPs, community services, and other involved parties as appropriate, so that home care support aligns with the wider plan around the person.

For individuals living with serious illness, we can provide supportive care focused on comfort, dignity, and routine, guided by the agreed plan and in communication with the people involved in the person’s care. This can include help with personal care, comfort measures, and practical support that helps the person remain settled at home. Where specialist clinical input is needed, we follow the established pathways so that the right professional support is involved.

Health-related support is delivered through careful planning, competent practice, and clear documentation. Records help ensure that support is consistent across visits, that concerns are not missed, and that communication is clear. We also support continuity by ensuring staff know the agreed plan and understand how to work respectfully with the individual’s preferences and needs. As a CQC registered provider, we operate with safeguarding, confidentiality, and safe practice as a baseline expectation across all health-related support we deliver.

Services include:

    Medication support (as agreed in the care plan): Prompting, supervision, and support with taking medication correctly, with recording in line with your procedures.
    Medication administration (where appropriate): Administration carried out only by trained staff and only where the person’s plan and arrangements allow for this, with accurate records maintained.
    Monitoring and reporting concerns: Noticing and reporting changes in wellbeing that may need follow-up, such as appetite changes, increased confusion, unusual drowsiness, or pain concerns.
    Support with hydration and nutrition routines: Helping the person maintain fluid intake and meal routines that support health, comfort, and recovery.
    Comfort-focused support for serious illness (as agreed): Practical support that helps the person remain comfortable and settled at home, aligned with the agreed plan and professional guidance in place.
    Reablement-style support (where part of the agreed package): Practical support that helps the person regain confidence and independence in day-to-day routines, within the scope of home care.
