About the Role
Role Summary
As a Care Coordinator, you will support the day-to-day delivery of adult home care by coordinating schedules, maintaining care plans, and working closely with care workers, people we support, families, and health and social care professionals. You will help ensure care is delivered safely, consistently, and in line with regulatory and confidentiality requirements.
Key Responsibilities
Coordinate and manage care worker schedules to support reliable service delivery.
Support new client onboarding: gather information, help set up care plans, and monitor ongoing needs.
Maintain accurate digital records and documentation, including care plans and scheduling updates.
Liaise with families, healthcare professionals, and other stakeholders to support joined-up care.
Carry out regular reviews and checks to support service quality and compliance.
Provide guidance and day-to-day support to care workers to promote good practice and teamwork.
Follow health and safety, safeguarding, confidentiality, and regulatory requirements at all times.
Occasionally support direct care in urgent or exceptional situations, when required.

Requirements
Qualifications and Education (Preferred)
NVQ Level 3 (or above) in Health and Social Care (or related field) is preferred.
Level 2 with relevant experience will be considered.
Applicants without the qualification who are willing to undertake training are welcome to apply.
Experience (Preferred)
Experience in a care setting is required.
Experience in coordinating, supervising, or leading care staff is desirable.
Experience supporting care planning, staff coordination, and liaison with professionals is highly desirable.
Experience working with older adults, people with disabilities, and people with complex needs is valued.
Candidates new to coordination but with strong care experience and readiness to step into leadership will be considered.

Legal and Compliance Requirements
Eligibility to work in the UK.
Enhanced DBS check required.
Knowledge of, and compliance with, health and safety standards, care standards, and confidentiality requirements.
Technology Skills
Comfortable using care management software and digital records systems.
Basic proficiency in Microsoft Office and general IT skills.
