About the Role

As a Care Coordinator at Serenity Care Circle Limited, you will play a key role in ensuring the delivery of high-quality care services. Your responsibilities will include:

  • Coordinating and managing the schedules of care workers to ensure efficient and effective service delivery.

  • Assessing new clients, developing personalised care plans, and monitoring the ongoing needs of existing clients.

  • Liaising with families, healthcare professionals, and other stakeholders to ensure cohesive care efforts.

  • Conducting regular reviews and audits to maintain high care standards.

  • Providing support and guidance to the care team, fostering a positive and collaborative work environment.

  • Ensuring compliance with all regulatory, health, and safety standards.

Requirements

Qualifications and Education: NVQ Level 3 or above in Health and Social Care or a related field is preferred. Candidates with a Level 2 qualification and relevant experience will also be considered. Those without this qualification but willing to undergo training are welcome to apply.


Experience: Proven experience in a care setting, with a preference for those who have held coordinating or supervisory roles. Experience in managing care plans, coordinating staff, and liaising with healthcare professionals is highly desirable. We value experience that includes working with elderly clients, individuals with disabilities, and those requiring complex care. Candidates new to care coordination but with significant experience in care settings and a readiness to step into a leadership role will be considered.

Organisational Skills: Exceptional organisational and multitasking skills to efficiently manage caregiver schedules, client appointments, and care plans.


Communication Skills: Outstanding verbal and written communication skills to effectively interact with clients, their families, healthcare professionals, and staff.


Interpersonal Skills: Strong ability to build and maintain trust-based relationships with clients, families, and team members.


Problem-Solving Skills: Proficiency in identifying issues and developing effective solutions in a timely manner.


Adaptability and Flexibility: Willingness to adapt to changing situations and the flexibility to work various shifts as needed to ensure seamless care delivery.


Attention to Detail: A keen eye for detail in creating and maintaining care plans, documentation, and compliance with regulations.


Physical Requirements: The role may require occasional direct involvement in client care, including emergency situations. A good level of physical fitness and the ability to respond effectively in such instances is necessary.

Legal and Compliance Requirements:


Eligibility to work in the UK is required.


An Enhanced Disclosure and Barring Service (DBS) check will be conducted.


Knowledge of and compliance with health and safety regulations, care standards, and confidentiality laws are imperative.


Professional Development: A commitment to continuous learning and professional development, including participation in training programmes provided by Serenity Care Circle Limited and external bodies to enhance leadership and care coordination skills.


Technology Skills: Proficiency in care management software, Microsoft Office, and general IT skills to manage digital records, communication, and scheduling efficiently.
